After scheduling an appointment to register a student, you MUST bring the following:

 

        Parent/ Guardian must accompany student during registration to provide accurate information and to sign necessary forms.

        Student must have transcript from the last school attended in order to be assigned proper grade level.

        A discipline report from previous school must be provided. If the child did not have any discipline issues, a statement from the school on their letterhead is sufficient.

        A copy of the student’s birth certificate

        Proof of Residency (utility bill, mortgage/rent receipt, etc.) Students who do not live in Haverhill cannot attend Haverhill High School without applying for School System of Choice through the Superintendent’s Office.

        Immunization Record and copy of most recent physical examination. TB screening may be required for immigrants from some foreign countries.

        Students who require special education support must have an IEP/504 at time of registration. This information must be provided prior to the day of registration to ensure special education staff is available to review the IEP/504.

        Students, whose home language is that other than English, are required to be language tested. This information must be provided prior to the day of registration to ensure that staff is available.

        Recommended: MCAS results for students transferring from a Massachusetts school.

 

On the day of your registration appointment, please follow the steps below:

 

·          Check into the Main Office to sign in and receive a visitor’s pass

 

·          Proceed to the Nurse’s Office for immunization review

 

·          Proceed to the Guidance Office for registration

 

Once this process is complete, a guidance counselor will work with the student/family to create an individual schedule.
